PMD 4.2.2 seems to have a bug: pom.xml sometimes is incorrect - empty pmd/file@name attribute and therefore how one knows where to fix the violation?

PMD 4.2.5 does not have this problem therefore you might consider releasing new version of maven pmd plugin that uses pmd 4.2.5

This was PMD bug 1988829 which was fixed in PMD 4.2.3
