[users@httpd] mod_auth_ldap not always challenging

Hi,

I have configured mod_auth_ldap on the apache Web server and it works 99% of
the time.  However, there are certain users that do not get an
authentication request.  This seems to be related to the proxy through which
the user passes.  Just by changing the proxy in the web browser a user was
challenged or not by the web server's mod_auth_ldap.

What could be the reason behind this problem?  Are there any resolutions for
this?  Obviously this is a serious security problem.
