At the moment the `AsciiDoc` and `Markdown` value types actually persist HTML; their only knowledge of those markup formats is in their factory methods when they eagerly convert the respective markup formats into HTML.

Instead, we should persist the markup format and then convert to HTML only lazily whenever rendering.

To store raw HTML, we'll rename the existing 'Markup' type to 'Html'.
