I'm using the following in a stylesheet in cocoon: 
 
 <xsl:apply-templates select="document('cocoon:/menu.xml')"/> 
 
It worked, till I updated to cocoon-2.1 and switched to Xalan 2.5.1. 
This problem was introduced with the fix to Bug #15200 .
