Looking at page 3 here->http://www.thomas-zastrow.de/st/examples/ 
nordland.pdf,
I'm wondering whether you have already considered including a keep- 
with-next="always" property on the headers. That would avoid stuff  
like the orphaned section-title on the bottom of the page.
