Brandon, can we put this in nodetool? We've run into this quite a bit and it would be helpful if it's in nodetool. I'd hate to make some of our tools call JMX endpoints + nodetool.

A possible use case: you bootstrap a node, it immediately dies before running cleanup.  Since the topology has effectively not changed, all you need is the token gone, but if you removetoken it unhelpfully tries to restore the replica count needlessly.

My hesitation with putting this in nodetool is that it should only be called post-0.8.3 (meaning CASSANDRA-2496) if:

* You got bitten by CASSANDRA-3259
* You got bitten by CASSANDRA-3351

In either case, both are resolved now so this would be a one-time thing to clear out the bad states.  This definitely shouldn't be called by a tool in any sort of automated way, and I don't want to give users the impression that it's a 'normal' thing to do, since that may paper over a real problem.
